Suspect a fracture when there is deformity, swelling, bruising, pain on movement or weight bearing, a grating sensation, or the person heard a crack. Do not try to straighten or realign the limb. Support it in the position found with padding, rolled clothing or a sling, and keep the person still and warm. Cover an open fracture with a sterile dressing without pressing on the bone. Call 112 or 999 for open fractures, fractures of the thigh, hip or pelvis, any suspected spinal injury, or if the limb is pale, cold or numb below the injury. Give nothing to eat or drink in case surgery is needed.
A sprain is a stretched or torn ligament at a joint, most often the ankle or wrist; a strain is the same injury to a muscle or tendon. The routine is RICE: rest the injured part, apply ice wrapped in a cloth for up to twenty minutes at a time, never directly on skin; compress gently with a bandage that does not cut off circulation; elevate above the level of the heart where possible. If the person cannot bear weight, the joint looks deformed or the pain is severe, treat it as a fracture and seek medical help; the two are often impossible to tell apart without an X ray.
For any blow to the head sit the person down, apply a cold compress to the swelling and watch them. Call 112 immediately for loss of consciousness however brief, drowsiness or confusion, repeated vomiting, a seizure, unequal pupils, clear fluid or blood from the ear or nose, weakness in a limb, a worsening headache, or a fall from height or high speed impact where the spine may be injured; in that case keep the head and neck still and do not move them. Concussion after a lesser knock still needs rest, no alcohol, no driving and no return to sport or physical work until cleared, with someone checking on the person for the next 48 hours.
